- Bank in Ringgit Malaysia One Hundred (RM 100) to MJ Life company bank account ( HSBC Account No: 353-177488-101 ), or pay by credit card, simply fill out the MJ Life Credit Card Charges Form. This amount is for the Business Development Kit.
- Fax application form, photocopy of the identity card or passport and bank in slip of deposit or Company Credit Card Charge Form to Fax No.: 603-77233377, and after all the MJ BDA Application Form C and identity card or passport photocopies should be sent to the company.

- 以现金方式汇入马币100令吉至美兆公司的银行户口，（ HSBC Account No:353-177488-101 ），或通过信用卡付款，只需填写美兆的信用卡收费表格。此款项是支付业务资料袋。
- 传真申请表格，身份证或护照影印本及银行存单或美兆信用卡收费表格至公司。传真号码： 603-77233377，后并将所有美兆BDA申请表格和身份证或护照的影印本寄回给公司。
