Conversion / Upgrading of the package of Medical Check Services
- While apply for conversion / upgrading, it is eligible to change the main membership card holder, the existing members of Medical Checks are not changeable, and the relationship of all the existing members with the new membership main card holder should follow all the terms and condition.
- The Flow of Application
- To apply with the former applicant's name, membership number, once again, fill in an "Application of MJ Medical Check Service (Standard Type)" form or "Application of MJ Gold Grade Medical Check Service" form.
- Bring along with all the particulars and documents of relevation members who need to be added and the existing forms of "Application of MJ medical Check Service (Standard Type)" or "Application of Gold Grade Medical Check Sevice" with all its copies to be submitted for the administration purpose of the company.
- Additional Fee: Just to pay the outstanding amount of conversion / upgrading.
- Th prohibition criteria of conversion / upgrading of Medical Check Service package.
- To reduce the Medical Check members. For example, from Type A Medical Check Services Package (6 memberships) convert to Type B Medical Check Services Package (3 memberships).
- Downgrading of the Medical Check Services Package. For Example, from Gold Grade Medical Services Package downgrade to Type A Medical Services Package.
